To make a request for disclosure, etc., please complete the necessary details on the request form (A), attach the identity verification documents (B), and, for requests regarding ‘notification of the purpose of use or disclosure’, enclose 800-yen worth of postage stamps and post the documents to the address below.

* Our response to your request for disclosure, etc., will be sent to the address stated on the identity verification document (please note that you must inform us of any change of address after submitting your request).

Please note that if the request is made by a representative/an agent, the following documents must be enclosed in addition to the identification documents listed above.

◎Where the applicant is the legal representative of a minor or a person under legal guardianship

  • Certified copy of family register: 1 copy
  • Copy of the representative’s driving license, copy of their passport or original copy of their certificate of residence: 1 copy

◎Where the applicant is acting as an agent under a power of attorney

  • Power of attorney bearing the principal’s signature and registered seal: 1 copy
  • Certificate of registered seal for the principal: 1 copy

If you are unable to print out the Request Form for Disclosure/Correction, etc. or the Power of Attorney, please contact the email address listed at the address below. We will send you a similar form to your preferred address.

【Fees and Other Matters】

For requests regarding notification of the purpose of use or disclosure, a flat fee of 800 yen in postage stamps will be charged to cover the actual cost of sending a written response (by registered post with proof of delivery). (No fee is charged for other requests, such as correction, addition, deletion, suspension of use, erasure, or suspension of provision to third parties). Please note that the fee amount is subject to change without prior notice due to social conditions or other reasons. Provided the request is complete, we will post our response within 14 days of receiving the necessary documents. Should it take longer than this, we will notify you in advance.
Please note that we may be unable to comply with your request if it falls under any of the following categories: ‘the request form contains incomplete information’, ‘the identity verification documents of the individual or their representative are incomplete’, ‘the fee is insufficient or unpaid’, or ‘the information does not fall within the scope of disclosure required by law’.

Should we acquire new personal information from you in connection with a request for disclosure, etc., we will handle it appropriately in accordance with the law.

6. Handling of Anonymized Information and Pseudonymized Information

When creating anonymized information and pseudonymized information, we will comply with the standards prescribed by law and implement appropriate security measures.

We will appoint a Personal Information Protection Manager and establish an appropriate internal personal information protection management system, and ensure the thorough protection of personal information through training on personal information protection for all directors and employees.
